چکیده

Objective: Endoscopic Submucosal Dissection (ESD) can be used to resect gastrointestinal diseases, but ESD costs long procedure time. It was reported that the rulers can serve as an ancillary device for shortening the procedure time. Thus, we aimed to identify a ruler assisted method performed by endoscopic nurses to shorten ESD procedure time and improve safety. Methods: A total of 30 patients indicated for ESD in our hospital from January 2016 to May 2016 were prospectively enrolled and randomly divided into control and experimental group, which received routine and ruler assisted intervention, respectively. We compared the demographic and clinical characteristics, procedure time and perioperative complications between two groups, analyzed the application of ancillary device assisted ESD. Results: There were 12 male and 3 female patients in control group. The diameter of the resected lesions was over 2.5 cm, and the maximal circumference was 9 cm. In experimental group, there were 6 male and 9 female patients. The diameter of the resected lesions was also over 2.5 cm, and the maximal circumference was 11 cm. The procedure time ranged from 45 to 90 min in control group, which ranged from 30 to 80 min in experimental group. There were no significantly statistical differences on the size of the resected specimen between the two groups. No obvious bleeding and perforation were observed. Conclusions: Our prospective data proved that the ruler assisted endoscopic device for ESD is safe and effective in shortening procedure time and avoiding mucosal injuries and complications.

Endoscopic submucosal dissection of gastric adenomas using the clutch cutter

AIM To evaluate the efficacy and safety of endoscopic submucosal dissection (ESD) using the clutch cutter (CC) (ESD-CC) for gastric adenoma (GA). METHODS From June 2007 to August 2015, 122 consecutive patients with histological diagnoses of GA from specimens resected by ESD-CC were enrolled in this prospective study. The CC was used for all ESD steps (marking, mucosal incision, submucosal dis...

Clinical Impact of Endoscopic Submucosal Dissection for Early Gastric Cancer

Aim: The aim of this study was to examine the safety and efficacy of endoscopic submucosal dissection (ESD) for early gastric cancer based on approximately 10 years’ experience. Methods: Endoscopic submucosal dissection was performed in 1272 patients with early gastric cancer (1657 lesions). We examined en bloc resection rate, curative resection rate, local recurrence rate, procedure time, and ...

Safety and Effectiveness of Endoscopist-Directed Nurse-Administered Sedation during Gastric Endoscopic Submucosal Dissection

BACKGROUND AND AIMS Endoscopic submucosal dissection (ESD) is routinely performed in treating gastric neoplasia and requires long-term higher levels of sedation. Endoscopist-directed nurse-administered sedation (EDNAS) has not been well studied in ESD. This study aimed to evaluate the safety and effectiveness of EDNAS for ESD. METHODS Patients treated with ESD for gastric tumors between 2013 ...

Endoscopic Treatment of Subepithelial Tumors

Gastrointestinal subepithelial tumors (SETs) are generally found during endoscopy and their incidence has gradually increased. Although the indications for the endoscopic treatment of patients with SETs remain to be established, the feasibility and safety of endoscopic dissection, including the advantages of this method compared with surgical treatment, have been validated in many studies. The ...

Safety and efficacy of endoscopic submucosal dissection for early gastric remnant cancers post-proximal gastrectomy with jejunal interposition

Introduction: Endoscopic submucosal dissection has been widely accepted as a standard treatment for early gastric cancers. However, endoscopic submucosal dissection for an early gastric remnant cancer post-proximal gastrectomy with jejunal interposition is not yet widespread. Case Series: A large, flat, elevated lesion was detected in an 83-year-old male (Case 1), whereas two separate flat, ele...
